AGREEMENT STUDENT FORM
This is an agreement between Licensor (CFILC) and Licensee, who is being licensed to use the named materials.
By purchasing a membership with CFILC, the student or instructor (Licensee) acknowledges that the system and name of I Liq Chuan, together with the I Liq Chuan Banner and logo and all materials both written and recorded and any derivative thereof are the intellectual property (The Work) of the Chin Family I Liq Chuan (CFILC).
CFILC agrees that the Licensee has a limited right to use the intellectual property described above subject to terms and conditions of the CFILC which may be revoked at the discretion of the CFILC. If this permission is revoked the Licensee must immediately cease and desist all use of said materials.
The Licensee agrees that CFILC retains all rights in any techniques and/or teaching methods and curriculum of the CFILC.
The student or instructor assumes full risk and responsibility for any and all damages, injuries or losses to his person or property, which he may sustain or incur in connection with conducting or participating in I Liq Chuan classes or workshops. He agrees that any member of the CFILC or promoters, operators and sponsors of the establishment, and their officers, agents or employees, shall not be responsible for any damages, injuries or losses to his person or property, which he may sustain or incur for any reason, cause or condition, whatsoever, including negligence. The student or instructor hereby releases and discharges each and all of them individually or otherwise of and from all claims, demands, rights or cause of action whatsoever, which he or his heirs, executors, administrators or assigns have or may have against them or any of them by reason of matter or incident, whether arising from their negligence or otherwise, in connection with his conducting or participation at any I Liq Chuan class or workshop.
The student or instructor agrees to the use of his name, photograph and voice in connection with the actual conduction of any tournament or event. In addition, he agrees to the use of his appearance, in the I Liq Chuan workshop or any of its functions, as part of a broadcast or telecast (live, taped, or rerun) or the exhibition of films and pictures of his participation, in any I Liq Chuan class or workshop or any its functions, without any compensation or with prior agreement with the CFILC.
I read and understand and hereby consent to the participation of the minor, attending the I Liq Chuan class or any of its aforementioned function, under the terms and conditions of this application, including the terms of release and consent not to sue contained therein. If under 18 years of age, this release and consent must also be signed by a parent or a legal guardian.
